2Send the Medical Release forms in addition to other required documentation to HME Specialists here, drop them off at one of our convenient branch locations or mail them to one of our branch locations.

Depending on the therapy, insurance required documentation may include: a signed physician prescription, applicable testing i.e. oxygen saturation test, and appropriate clinical documentation
