Hospice Coordinator

Applicant chosen must show proof of full vaccination or provide a written religious or medical exemption and agree to weekly testing.

Our Hospice Coordinators are responsible for assisting LTC/Hospice patients and facilities to receive medical equipment supplies as needed.

Duties include:

  • Assist long term care and hospice agencies in selection of correct billing codes and equipment or service selections
  • Answer and distribute incoming telephone calls
  • Accept inbound calls, listen to requests from the callers and determine how to move forward with the request.
  • Complete requests and ensure that the callers request was addressed.
  • Speak with customers by phone or in person and receive orders for delivery, pick-up, discontinuance, maintenance, or changes in service.
  • Obtain complete and accurate information for Patient Demographics forms.
  • Work in DMETrack and Hospice Cloud to open, close and distribute orders.
  • Participate in new patient conferences. (Huddle)
  • Pull and distribute faxes accordingly.
  • Complete assigned projects.
  • Participate in Customer Service on-call after hours duties
  • Ensure accuracy of referral logs.
  • Assist with month end processes, invoicing, and billing practices.
  • Solicit sale of new or additional services.
  • Document complaints of service and equipment failures.
  • Speak with dissatisfied customers and coordinate the correction of the complaint and ensure service recovery.

Preferred skills include, experience with health plans or medical information, insurance verification, prior auth processing, strong organizational skills, ability to multitask and have strong work ethic and ability to communicate at all levels.
